- Original passport, valid at least 6 months, with 2 blank pages
- CNIC copy (front & back); B-Form for children under 18
- 4 recent passport-size photographs, white background
- Meningitis (ACYW) vaccination certificate
- Polio vaccination certificate
- Nikah Nama / FRC for families, where applicable
